TURKISH BUSINESS FORUM (TBF)
MEMBERSHIP APPLICATION AND COMPLIANCE CONSENT FORM

I, [Full Name], hereby apply for membership with the Turkish Business Forum (TBF) and declare, acknowledge, and consent to the following terms:

1. Membership Conditions and Ongoing Compliance

I confirm that I have fully read, understood, and accepted the TBF Membership Agreement, Disciplinary Regulations, Ethical Principles, and all relevant policies and procedures.
I undertake to continuously meet the “Fit and Proper Person” criteria throughout my membership.
I accept that I will comply with any future changes made by TBF to its membership requirements and policies.

2. Adherence to Ethical Principles

I undertake to conduct myself in accordance with the following ethical principles in all business relations and TBF activities:
• Integrity and reliability
• Compliance with applicable laws and regulations (including Companies Act 2006, Equality Act 2010, Competition Act 1998, Data Protection Act 2018, Bribery Act 2010)
• Commitment to moral values and corporate reputation
• Transparency and fair competition
• Avoidance of conflicts of interest

3. Notification Obligations

I undertake to notify TBF in writing within 30 days in the event of:
• Any disciplinary sanction or enforcement action
• Any criminal conviction (unspent offences)
• Bankruptcy, debt management plans, or insolvency arrangements
• Disciplinary actions imposed by professional bodies or regulators
• Loss of licence, authorisation, or certification

4. Acceptance of the Disciplinary Process

I accept that I may be subject to disciplinary investigation if I act contrary to TBF’s Ethical Principles or disciplinary provisions.
I agree to respond within reasonable timeframes and fully cooperate during investigations.
I accept the decisions of the Disciplinary Committee and understand that any appeals must follow the prescribed procedures.

5. Emergency Termination of Membership

I accept that I may be immediately and permanently expelled from membership in cases of serious violation, including:
• Judicial Actions: A lawful search warrant or arrest order issued against me
• Serious Criminal Conduct: Verified involvement in fraud, embezzlement, bribery, corruption, organised crime, terrorist financing, or similar serious offences
• Final Criminal Conviction: Any finalised conviction, particularly involving financial crimes, organised crime, terrorist financing, sexual offences, or violent crimes
• Security Threat: Behaviour that threatens the safety of TBF, its members, or its employees

6. Consent for Sanctions and Publication

I accept any disciplinary sanctions that may be imposed on me (warning, financial penalty, temporary or permanent expulsion, etc.).
I explicitly consent to:
• Publication of disciplinary decisions and membership termination on the TBF website and communication channels
• Such publications remaining accessible for at least 3 years
• Publication of the grounds for termination in emergency cases as stated in Section 5

I acknowledge that I understand the potential impact of such publications on my personal and professional reputation.

7. Reasonable Adjustments

If I require reasonable adjustments due to health conditions, disability, or protected characteristics under the Equality Act 2010, I agree to notify TBF in writing.
I accept that TBF may provide reasonable adjustments and that these may be considered in disciplinary processes.

8. Data Processing Consent

I consent to the collection and processing of my personal data by TBF for membership administration, disciplinary processes, and legal compliance.
I consent to the sharing and publication of my personal data within the scope of disciplinary decisions.
I confirm that I understand my rights under the Data Protection Act 2018 and GDPR.

Additionally:
• I agree to receive communications, updates, newsletters, invitations, and relevant information from TBF via email or other preferred channels.
• I consent to my information being shared with other TBF members, business partners, and sponsors in accordance with TBF’s privacy and data-sharing policies, unless I request otherwise in writing.
• I understand that I may cancel my membership at any time in accordance with TBF’s membership cancellation policy.

9. Declaration and Undertaking

I declare that I fully understand and voluntarily accept all provisions in this document.
I unconditionally grant TBF the authority to administer disciplinary processes, issue sanctions, publish decisions, and process my personal data as required.

I acknowledge that I have read, understood, and approved all terms of this consent form.
